APPLICATION FORM FOR DUPLICATE CERTIFICATE
For a
set (five copies) of Academic Transcript :
1. xe-rox copy of marksheets/gradecards of all previous examinations,
relevant to such transcript. Without xe-rox copy of Marksheets, Form will
be cancelled. The form must be signed by the candidates and may be collected by candidate
or duly authorized person after minimum fifteen days.
2. Rs. 300/- only
For a
duplicate certificate :
1. marksheet/gradecard of Final examination.
2. Rs. 100/- only

In each case candidate may collect the form from Controller of
Examinations,JU’s dept and submit the form, duly filled in, Cash
section before 3p.m
